Conclusion: Glutaraldehyde treatment improved the tensile material properties at the fascicle and fibril level. The similar findings at two structural levels of the tendon hierarchy demonstrate that cross-linking is a principal element of tensile tendon strength. Combining macro- and nano-scale mechanical testing allows for new insight into fundamental structural determinants of tendon mechanical properties. Methods: 5 different test protocols for SLS and 1 test protocol for LSD was being tested twice by 2-3 observers on all together 98 subjects. Results: Intertester reproducibility was generally high, and higher than intratester reproducibility, due to variations in execution between trials. When the subjects were scored on a scale from -15 to +15 while performing 15 repetitions intertester SEM were compared to and intratester SEM were compared to for SLS and LSD, respectively. When subjects were further instructed to maintain the knee over 2. toe and to stabilize the upper body during SLS, intertester and intratester SEM were and , respectively. Conclusions: Using the latter protocol the SLS can be used as a reproducible clinical test to determine betweensubject variation frontal plane knee movements. If the test is used to evaluate small to moderate differences in knee movement, the variation between trials may be too excessive. 18. Nano-Science Center, University of Copenhagen, Denmark Institute of Sports Science and Clinical Biomechanics, University of Southern Denmark, Odense, Denmark Introduction: Tendons possess impressive tensile strength and collagen tissues are thought to obtain crucial mechanical integrity from intermolecular cross-linking. However, insight into the nano-scale principles of tendon strength, including the importance of cross-linking, is insufficient. Atomic force microscopy (AFM) allows for the investigation of the nano-scale mechanical properties of biological tissues. Materials & methods: Cross-linking was induced in rat-tail tendon fascicle segments by glutaraldehyde (GA). Untreated segments served as control. Mechanical properties were examined at both the macroscopic fascicle and nanometer fibril levels by micromechanical testing and atomic force spectroscopy, respectively. Results are reported as mean±sd. Results: At the fascicle level GA treatment produced markedly improved mechanical properties of the tendon fascicles (yield stress: 39.3±16.0 MPa (GA) vs. 8.0±1.9 MPa, p<0.01; yield strain: 10.6±2.5 % (GA) vs. 3.7±0.4 %, p<0.01). AFM spectroscopy revealed comparable improvements occurring at the fibril level (peak force 2622±1441 pn (GA) vs. 1379±596 pn, p<0.001). A similar pattern of changes was observed with AFM nano-indentation. Med baggrund i DIMS 50 års jubilæum har vi sat alle sejl til, for at kongressen forhåbentlig kan gå ind i historien. Dette fejres i øvrigt ved et symposium, hvor 3 af selskabets koryfæer vil præsentere historie og videnskab. Vi har inviteret Dansk Selskab for Artroskopisk kirurgi & Sportstraumatologi, Dansk Selskab for Skulder-Albue kirurgi, Antidoping Danmark samt Dansk Selskab for Fysisk Aktivitet og Sundhed til at arrangere symposier ved kongressen. Det er en stor glæde, at dette samarbejde har ført til så fornemt resultat. Mange har efterlyst muligheder for at tilegne sig praktiske færdigheder ved kongressen, hvorfor vi hver dag starter med sideløbende praktiske kurser og work-shops. Der er indkommet en række abstracts, som fremlægges ved 3 sessioner i løbet af kongressen. De 6 bedste, vurderet ved en uvildig tværfaglig vurdering, er udvalgt til at konkurrere om de 3 præmier ved kongressen. Der er ikke fundet plads til postere. Internal extensor moments, adductor moments, and outward rotator moments were dominant around the hip joint in this time period. Conclusion: The present results show a dependency on the medial hamstrings to counteract the external valgus and outward rotating moments occurring around the knee joint during the sidecut manoeuvre, and that hip extensors, outward rotators, and hip adductors are the most loaded muscle groups during this crucial early part of the sidecut. Attention to these muscle groups may be important in prevention programs and rehabilitation regimes involving female handball players. 17. In combined lesions the medial collateral ligament rupture can be treated non-operatively combined with reconstruction of the central pivot. In cases of multiple ligament injury or Grade III non-operative treatment of the medial collateral ligament lesion may lead to varus instability or rotatory instability. Material: In the period from Januar 2004 to December patients with grade 3 medial instability were treated with primary or secondary medial collateral ligament reconstruction. Median age was 35 years (range 14 to 64 ys), 50 were males. 8 were revision cases. Method: All had reconstruction of the medial collateral complex using hamstring grafts. Through a oblique approach the pes anserinus was exposed and the Semitendinosus tendon harvested utilising an open tendon stripper. The distal incersion was kept intact. The posteromedial corner of the tibia was identified through the same approach. Through a small incision at the medial femoral epichondyle the proximal incersion point of the MCL was identified distal to the fibres of the m.quadricipitis obliquus and posterior to the medial patellofemoral ligament. Drill-holes through proximal tibia and femur were done and a reconstruction of the medial collateral ligament superficial layer and the postero oblique fibres were performed. All were available for follow-up more than 12 mths post-op and were examined using the IKDC form and subjective KOOS score. 16. KINETIC ANALYSIS OF KNEE AND HIP JOINT LOADING DUR- ING SIDECUTTING IN HANDBALL IMPLICATIONS FOR PREVEN- TION AND REHABILITATION AF- TER ACL-INJURIES Jesper Bencke, Christina Krogshede, Jeppe N. Jensen, Derek Curtis Ganganalyse laboratoriet, Ortopædkirurgisk afdeling, Hvidovre Hospital Introduction: Injuries to the anterior cruciate ligament (ACL) has previously been reported to occur early in the eccentric part of a handball sidecut manoeuvre with the knee near full extension, outward rotation and with increased external valgus moments. Three-dimensional kinetic analyses of the sidecut manoeuvre in female handball players have not yet been performed. Methods: Twentyfour young female handball players (age 15-18yrs) agreed, with their parent s consent, to participate. Five repetitions of each player s individual sidecut manoeuvre were investigated using an 8 camera Vicon 612 system and an AMTI forceplatform. Net moment in three planes around the knee and hip joints were calculated, and the net moments during the first 30% of the contact phase was selected as outcome measures. GROWTH HORMONE SUPPLE- MENTATION UP-REGULATES COL- LAGEN EXPRESSION IN HUMAN MUSCLE AND TENDON Døssing S, Heinemeier K, Holm L, Reitelseder S, Koskinen S, Schjerling P, Kjaer M Institute of Sports Medicine, Bispebjerg Hospital and Faculty of Health Sciences, University of Copenhagen, Copenhagen, Denmark Introduction Growth Hormone (GH) potentially influences connective tissue in muscle and tendon. Methods We studied the effect of 14 days of recombinant human GH (rhgh) supplementation and exercise, on the expression of mrna for Insulin-Like Growth Factor-I (IGF-I), collagen I and collagen III in human skeletal muscle and tendon, in a double blind, randomized, placebo-controlled crossover design. 10 male subjects received rhgh (0.33 0,50 mg/kg/day) or placebo separated by 4 months. One-legged exercise (10 10 reps at 70% of 1 RM) was performed on the last day of the supplementation period, and 24 hours later tissue was sampled bilaterally from the vastus lateralis muscle and the patella tendon. mrna expression was measured using real-time RT-PCR. Results and Conclusion rhgh administration resulted in a 8-fold rise in circulating GH and a 3-fold increase in circulating IGF-I. In addition tendon and muscle connective tissue mrna expression for IGF-I increased significantly with GH (p<0.05). Furthermore, muscle and tendon collagen I and III increased significantly in response to rhgh supplementation (p<0.05). The results indicate a role for rhgh via IGF-I in increasing collagen content in connective tissue of both tendon and skeletal muscle. 14. Materiale og metoder: Der er lavet et databaseskema til registrering af subjektive og objektive parametre. Dette skema vil blive præsenteret sammen med preliminære resultater. At baseline, the majority of players (87%), had color Doppler flow in at least one scanning position. After match the percentage of the knee complexes involved, did not change. Color fraction increased significantly in the dominant leg at the tibial tuberosity, single players had a significantly higher CF after match at the tibial tuberosity and in the patellar tendon both before and after match. Painful tendons had highest color Doppler activity. Conclusions Most elite badminton players suffered from pain in the anterior knee tendons and most had intratendinous Doppler activity both before and after match. High levels of Doppler activity were associated with self-reported ongoing pain. 12. Hypothesis Ultrasound with Doppler is a sensitive parameter in the diagnosis of jumper s knee Design Cohort study Methods Forty patients with jumper s knee were interviewed about basic characteristics, sports, training, former treatment, symptoms and the impact of jumper s knee on daily life. They filled in VISA and KOOS questionnaires. Isokinetic muscle strength was measured. The pelvis and lower legs were examined. The anterior knee tendons were examined with ultrasound, including color Doppler. The amount of intratendinous color Doppler activity was measured as the color fraction. Results The most frequent findings in jumper s knee patients are: Activities: Being athlete with many and often long training hours. Symptoms: Shooting pain in the anterior knee tendons of the dominant knee and/or pain that increases in intensity during activity. Painful walking of stairs. Clinical evaluation: Pain at palpation in the tendon immediately superior or inferior to the patella. Findings at the tibial tuberosity are more equivocal. Ultrasound Doppler: Color Doppler activity at the site of pain with a color fraction value higher than 10% and/or a mean RI below Conclusions The history, the symptoms and the clinical evaluation are not unique for jumper s knee. Ultrasound Doppler is highly sensitive in diagnosing jumper s knee by detecting intratendinous hyperemia. Introduction: Approximately 1/3 of the training performed by the Danish National Team rowers consists of land based training in rowing ergometers. Consequently, the type of rowing ergometer may have strong influence on rowing performance and risk of injury. The aim of this study was to investigate force production during rowing in the commonly used Concept2 Rowing Ergometer with and without slides, to evaluate if placement of the ergometer in slides may reduce the risk of musculo-skeletal overuse injury. Material and Method: 14 male and 8 female National Team rowers performed two rowing trials at % of maximal power output with and without slides in a randomized order. A strain-gauge measured the force production (1000 Hz) and a potentiometer measured the displacement of the handle (1000 Hz). Paired t-tests were used for statistical comparisons. Results: During slide-rowing the male rowers increased the stroke rate from 25.9 ± 1.2 to 28.7 ± 1.7 strokes/min. (p<0.001) and decreased the peak force from 887 ± 87 N to 811 ± 74 N (p<0.001) compared to the stationary rowing trial. A similar but less pronounced response to slide-rowing was observed in the female rowers. Conclusion: Placement of the rowing ergometer in slides decreased the peak-force production in each rowing stroke while notably the same external power output was maintained. Consequently, rowing in slides can be hypothesized to reduce the risk of overuse injury without compromising the training efficiency and rowing performance of the rowers. Prospective controlled studies are required to test this hypothesis. 11. ULTRASOUND DOPPLER OF THE ANTERIOR KNEE TENDONS IN ELITE BADMINTON PLAYERS COLOR FRACTION BEFORE AND AFTER MATCH Koenig MJ, Torp-Pedersen ST, Boesen MI, Holm CC, and Bliddal H. Abstract Background Anterior knee tendon problems are seldom reported in badminton players although the game is obviously stressful to the lower extremities. Hypotheses Painful anterior knee tendons are common among elite badminton players. The anterior knee tendons exhibit color Doppler activity. This activity increases after match. Painful tendons have more Doppler activity than tendons without pain. Study design Cohort study Methods Seventy-two elite badminton players were interviewed about training, pain and injuries. Color Doppler was used to examine tendons of 46 players before and after match. Intratendinous color Doppler flow was measured as color fraction. The tendon complex was divided into three loci; the quadriceps tendon, the proximal patellar ligament and the insertion on the tibial tuberosity. Results Of the 72 players, 62 players had complaints from 86 tendons in the lower extrem- (Fortsættes på side 34) Side 33

Data form these groups were extracted from ACL registry with respect to epidemiology, and Tegner and KOOS scores preoperatively and at 1 year follow-up. RESULTS patients fitted the selection criteria. 258 patients had full follow-up data. Both KOOS and Tegner scores at 1 year follow-up was significantly higher in patients with sport injuries compared to non-sport injuries. The KOOS subscore with the highest difference between the two groups was Sport/ leisure. Tegner score was 5.3 and 3.7 in athlete patients and non-athlete patients respectively. CONCLUSION. This study demonstrates that patients injured in sports activities achieved better activity scores and subjective pain and function scores than patients injured in non-sports activities. This could be caused by athlete patients to be more motivated in their rehabilitation effects. Further studies that elucidate compliance are necessary. 9. INCREASED MYOGENIC PRE- CURSOR CELL NUMBER IN HU- MAN SKELETAL MUSCLE WITH 12 WEEKS OF LIGHT MUSCLE LOAD- ING Abigail L. The response of this myogenic precursor pool to very light muscle loading is however unknown. We therefore designed a 12 week training protocol where volunteers trained one leg with a heavy load (H) and the other leg with a light load (L). 12 young healthy men (mean age 25 ± SD 3 yrs) volunteered for the study. Muscle biopsies were collected from the m. vastus lateralis of both legs before and after the training period. Satellite cells were visualised by neural cell adhesion molecule (NCAM) immunohistochemistry. Compared to the pre training values, there was a significant increase in the number of NCAM+ cells (per myonuclear number) post training in both groups (H: from 4.16 ± 1.01 to 5.32 ± 1.22; L: from 4.01 ± 0.78 to 4.72 ± 1.72; mean ± SD, p < 0.05). Århus C, Denmark BACKGROUND. Clinical outcome after anterior cruciate ligament (ACL) reconstruction is dependent on multiple factors. Cause of injury and patient goals to return to sports activities could potentially affect clinical outcome. This study presents 1 year follow-up clinical outcome data from the Danish ACL registry in patients injured by sport activities compared to patients injured in non-sport activities. METHODS. The Danish ACL Registry contain anamnestic, objective and operative data. The patients register KOOS and Tegner score preoperatively and at follow-up. In the present study we selected patients injured in soccer, team handball and basketball to represent athlete patients. There was no difference in fascicle diameter between the corticosteroid treated group and the control group. Conclusions. Corticosteroid administration reduces the tensile strength of tendon collagen fascicles 3-days post injection. This corticosteroid-associated reduction might render an already unhealthy tendon closer to its failure limit. 7. However, positron emission tomography (PET) may offer an alternative by 3D imaging of contraction-induced glucose uptake in muscle tissues. Recent technological advances have increased PET resolution, and the purpose of the present study was therefore to investigate activation of the plantarflexors during voluntary and stimulated muscle action by use of PET. Materials & Methods: Five males performed a regimen of unilateral intermittent isometric contractions on two separate days (electrical stimulation of the m. gastrocnemius medialis (GM) and voluntary plantar flexor contractions respectively). A separate group of subjects (n=6) participated on two separate days and performed dynamic unilateral concentric and eccentric plantarflexor contractions respectively. During all protocols 240 MBq of [18F]-fluoro-deoxy-glucose was injected after 5 min., and subsequent to contractions the legs of the subjects was PET scanned. Results: The stimulated GM displayed increased glucose uptake compared to adjacent passive muscles (P<0.001) and to the resting GM (P<0.001). On the voluntary isometric day GM and SOL (Soleus) exhibited increased glucose uptake when compared to contralateral resting muscles (P<0.001), however, no significant increase was observed in GL (m. gastrocnemius lateralis) compared to the resting GL (P=0.13). Between the days with eccentric vs. concentric contractions inter-muscular differences in activation were observed (P<0.05) Discussion: The present study demonstrated non-uniform activation of the plantarflexor muscles during different submaximal contractions. Furthermore, it was demonstrated that percutaneous stimulation of the GM activated the entire muscle in a uniform manner, and no glucose uptake was observed in adjacent muscles. It provides a comprehensive evaluation of a person s psychological health status and resilience. The profiles are subsequently linked to a set of written self-help programs based on cognitive-behavioural principles that are addressed to the particular kinds of problems revealed in the profile. The patients were investigated according our Shoulder Package (Xray, Ultrasound, MRI, Physiotherapy). The most frequent diagnosis was impingement 174. Only 72/387 patients underwent surgery. RESULTS Severe stress, anxiety or depression was seen in one third to one fourth of the patients. 4% had no hope for the future. A total of 94 patients clearly might benefit from some sort of psychiatrically intervention: Anti depressive medication and/or psychotherapy or our cognitive-behavioural self help program. CONCLUSION Most of the cases of depression in our population might have remained unrecognised if PPQ was not used. It seems very important to identify the psychological functioning and personality in this category of shoulder patients in order to provide an enhanced and more comprehensive strategy of treatment. Special attention regarding the timing of surgery must be taken towards patients with depression or other severe psychological symptoms. 6.CORTICOSTEROID ADMINISTRA- TION ALTERS THE MECHANICAL PROPERTIES OF ISOLATED COLLA- GEN FASCICLES. Introduction. Overload tendon injuries are frequent both in recreational and elite sports. The optimal treatment strategy remains unknown, but local administration of corticosteroids is one common treatment option. The direct effects of the corticosteroid administration on the tissue are not fully understood. The purpose of the present study was to examine the effects of corticosteroid short-term administration on the biomechanical properties of tendon collagen fascicles. Methods. A total of 24 Wistar male rats were divided into two groups; in the corticosteroid group the animals were injected in the tail tendon with methylprednisolone acetate, 1.0 ml of 40 mg/ml mixed with 1.0 ml 9 % saline (n=12); and the control group was injected with 9 % saline (n=12). Three days after the injections the animals were sacrificed and the tails collected. Single individual collagen fascicles were isolated from the structure, preconditioned and subsequently underwent displacement to failure at 0.07 mm/s. Results. Corticosteroid administration significantly reduced tensile fascicle strength compared to sham treatment (10.5 (SD 0.8) MPa vs (SD 0.5) MPa, P 0.05). Fur- (Fortsættes på side 30) Side 29

Conclusion: OC seems to have a general inhibiting effect on protein synthesis and especially an inhibiting effect on tendon synthesis in response to exercise either directly or indirectly by a reduction of the bioavailability of IGF-I. 4. HABITUAL LOADING YIELDS PATELLAR TENDON HYPERTHO- PHY Couppé C, Kongsgaard M, Hansen P, Kjær M, Magnusson S.P. Institute of Sports Medicine, Bispebjerg Hospital and Faculty of Health Sciences, University of Copenhagen, Denmark Introduction: The average stress (force/area) of a tendon is determined by the tendon cross-sectional area (CSA) and tendon force. Thus, a larger tendon CSA will reduce tendon stress and protect against injury. It is unknown if human tendons hypertrophy with long-term habitual loading. Purpose: To examine bilateral patellar tendon CSA in subjects with a substantial bilateral lower-extremity strength difference due to prolonged sport-specific loading. Material and Methods: Six young male elite fencers & badminton players (22±5 yrs, 73±6 Kg) with 20 % difference in side-side isometric knee extension strength (237 Nm vs. 191 Nm, p<0.05) were included. All were healthy and without knee pathology. Axial MRI along the patellar tendon was obtained. Subsequently, tendon CSA (mm2) at the proximal, middle and distal-tendon regions was determined in a blinded analysis. Results are reported as mean±sd. Results: Distal and proximal-tendon-csa, but not middle-csa, were greater on the strong-side compared to the weakside (distal: 157±54 vs. 133±42; middle 103±44 vs. 90±11; proximal 122±24 vs. 97±21; P<0.05). Distaltendon-CSA was greater than middle and proximal-tendon-csa on both the strong and weak-side CSA (P<0.05). For a given common force, stress was lower on the strong-side (49±11 MPa) compared to the weakside (61±21 MPa)(P<0.05). Maximal stress at the middle-tendon was similar for the strong (71±23 MPa) and weak-side (71±21 MPa). Conclusion: We demonstrate a regional variation in CSA along the patellar tendon, which markedly influences tendon stress. The increased proximal and distal-tendon-csa on the strong-side suggest that increased habitual loading results in regionspecific tendon hypertrophy. 5. Results: All groups improved in VISAp and VAS from 0 to 12 wks (p<0.05). The VISA-p andvas improvements were maintained at follow-up in ECC and HSR but deteriorated in CORT (p<0.05). Tendon swelling decreased in CORT and HSR (-13+2 & -12+2%, p<0.05) but was unchanged in ECC. Vascularisation decreased in CORT and HSR ( & -45+6%, p<0.01) but was unchanged in ECC. At follow up, HSR were more satisfied than CORT and ECC. Mechanical properties did not differ in healthy and injured tendons and remained unchanged in all groups. Conclusion: CORT had excellent short term but poor long-term clinical effects. HSR have superior short and long-term effects. Tendon mechanical properties are unaffected by tendinopathy and do not change with either treatments. 3. Material and Methods: Healthy young women, OC-users (n=11) and control (n=12), were studied 24h following one-legged kicking exercise (EX) for 60 min at 67 % of Wattmax. Stabile isotopes (13C-proline and 13Cleucine) were infused followed by muscle and tendon biopsies after six hours. Microdialysis catheters were placed anterior to the patellar tendons, and interstitial fluid was analyzed for the amino-terminal propeptide of type-i collagen (PINP), a marker of tendon collagen synthesis. Results: An exercise-induced increase in tendon synthesis was observed in control (PINP: Rest: 25.3 ± 11 (mean ± SD) ng ml-1 vs. EX: 48.3 ± 28 ng ml-1, P < 0.05), but not in OC-users (EX: 27.4 ± 16 ng ml-1 vs. Rest: 31.5 ± 20 ng ml-1). The preliminary isotope data indicates a general lower protein synthesis rate in all tissue fractions by OC. Insulin-like growth factor I (IGF-I) both in serum and in the interstitial tendon tissue fluid was significantly lower in OC-users compared to control, whereas the level of Insulin-like growth factor bindingproteins (IGFBPs) were significantly (Fortsættes på side 28) Side 27

It was the purpose of this study to investigate if warm-up with balance exercises improves postural control during quiet standing and landing more than warm-up without balance exercises. Material & Method: Twenty healthy subjects (mean ± 1 SD, 27.2 ± 3.6 yrs) were examined twice (15-min standardised warm-up with and without balance exercises included) in a crossover study with two weeks between visits. The order of warm-up procedure was randomised. At each visit, postural control during quiet standing (single limb stance) and single-legged landing was measured pre and two times post warmup using a biomechanical force plate. During quiet standing, postural control was quantified as the total excursion of the center of pressure (sway length), and during landing as the time to stabilisation in the anterior-posterior and medio-lateral direction (TTSant and TTSlat, respectively). Two-way repeated measures ANOVAs were used to examine main effects of time and type of warm-up on the dependent variables. Results: Both types of warm-up improved postural control (i.e. reduced sway length) during quiet standing to the same extend over time (P = 0.001). Postural control during landing increased (i.e. TTSlat decreased while TTSant remained the same) more over time after warm-up with balance exercises compared to warm-up without balance exercises (P = 0.009).
