1 Title: Measuring adolescent alienation: Criterion-related construct validity and the association with health related outcomes SUMMARY Psychological alienation (hereafter: alienation) is a serious strain for a young person. It is characterised by feelings such as powerlessness, meaninglessness, social isolation and normlessness. Several scholars have emphasized that feeling related, socially supported and competent are basic needs for a positive psychosocial development. Alienation compromises these needs. Experiencing alienation may be particular serious in adolescence where the reciprocal exchanges between the young person and the social environment play an important role in the psychological and social changes characterizing this stage of life. The basic assumption of this thesis is that alienation is an important concept in the study of adolescents health and health related behaviour. It is therefore important with sound scales for the measurement of adolescent alienation. Most research on alienation as a psychological concept is founded in Melvin Seeman s conceptualization and includes some of his six alternative variants of alienation. Prior studies have found alienation to be predictive of alcohol use, drunkenness, truancy, drug use and other kinds of risk behaviour among adolescents. The hypothesis of this thesis is that alienation increases the risk for adverse health related outcomes such as high symptom load and disadvantageous health behaviour, in this case physical inactivity. The association between alienation and health related outcomes such as symptom load and physical inactivity is, however, only sparsely investigated among adolescent. Additionally, the measurement of alienation meets several challenges. Not all alienation scales used in adolescent research have been validated on an adolescent sample and despite their common conceptual background there is considerable heterogeneity in measurement and validation. Few scales have been validated in relation to the increasing demands to construct validity. The overall objectives of this thesis were therefore, firstly to develop and validate alienation scales in accordance with Seeman s conceptualization for use in adolescent health research. Secondly to

2 investigate the association between alienation and selected health related outcomes: 1) symptom load and 2) physical inactivity. More specifically the aims were: (1) to construct and validate a simple and global index of alienation. (2) To develop specific scales for different variants of alienation among adolescents and investigate the criterion-related construct validity of these scales. (3) To examine the association between the outcome symptom load alienation, measured by the global index, among adolescents. (4) To examine the association between the outcome physical inactivity and different variants of alienation, measured by specific scales, among adolescents. The thesis is based on data from three Danish contributions to the cross-national survey Health Behaviour in School-aged Children (HBSC), an international cross-sectional WHO collaborative study. The overall purpose of the study is to increase understanding of young people s health and lifestyle in their social context. The HBSC study comprises repeated cross-sectional questionnaire surveys among 11, 13 and 15-year olds in a random sample of schools. The analyses are based on Danish data from the HBSC-study 1998, 2006 and 2010 and include the year old (1998: n=5205) and year old (subsample in 2006: n=2314; 2010: n=3083) pupils. The initial global index of alienation was constructed and validated in relation to four criteria: 1) theoretical foundation, 2) inter-correlation between items, 3) correlation between the items and the outcomes and 4) no differential item effect (DIE). The following sub-studies of the thesis applied item response theory (IRT) to examine the psychometric properties of the scales for different variants of alienation. In IRT one assumption is that it is possible to measure a concept which cannot be observed directly (a latent variable referred to as Theta(θ)) by a range of items reflecting its consequences. The alienation variants measured in this thesis are such latent variables. In the examination of the scales psychometric properties, I used Rasch models (RM) and graphical loglinear Rasch models (GLLRM) to ensure the criterion-related construct validity. I also used GLLRM to generate θ estimates adjusted for local dependency (LD) and differential item functioning (DIF) for each individual. These θ estimates were subsequently used in the analyses of the association between alienation and physical inactivity. Main findings: (1) The global index of alienation measured aspects of alienation and comprised three indicators of alienation: helplessness, feeling left out of things and lack of confidentiality with parents. Measured by this index there was a graded and significant association between alienation and both high psychological and physical symptom load. (2) It was possible to construct scales for two of Seeman s alienation variants: normlessness and social isolation, based on items originally

3 developed for other purposes in the HBSC 2006 (subsample) questionnaire. The scales contained some DIF and LD, however, only to a limited degree. Both scales fitted a RM when using GLLRM and are essentially construct valid, essentially objective and sufficient. (3) It was possible to develop scales for four variants of alienation based on items specifically chosen or developed to measure alienation in the HBSC 2010 questionnaire: powerlessness, meaninglessness, social isolation and normlessness. The scales included face valid items and were in accordance with Seeman s conceptualization. All four scales contained some DIF and LD but again to a limited degree which could be taken into account using GLLRMs. The scales are essentially construct valid, essentially objective and sufficient. (4) The four variants of alienation: powerlessness, meaninglessness, social isolation and normlessness were significantly associated with physical inactivity. The effect of the three scales of powerlessness, meaninglessness and normlessness on physical inactivity was non-linear. For both powerlessness and normlessness scatterplots of the total effect (OR) against the alienation variable showed a stable association at the lowest θ values where after the ORs increased with an increasing intensity. In relation to meaninglessness the association was j-shaped. This thesis suggests that it is possible to develop construct valid alienation scales for use in adolescent health research. It also suggests that alienation is associated with selected health outcomes such as symptom load and physical inactivity. The alienation variants studied in this thesis may contribute to an increased insight into psychosocial processes which influence health in youth. The concept of alienation may therefore play a role in future research on adolescent health and health behaviour.

4 DANSK RESUMÉ Fremmedgørelse i psykologisk betydning (herefter: fremmedgørelse) er en alvorlig belastning for et ungt menneske. Fremmedgørelse er karakteriseret ved følelser såsom magtesløshed, meningsløshed, social isolation og normløshed. Flere forskere har fremhævet at det, at føle sig relateret til andre, at føle sig støttet socialt og at føle sig kompetent, er basale behov, der er vigtige for en positiv psykosocial udvikling. Fremmedgørelse kompromitterer disse behov. Da samspillet med sociale relationer og omgivelser spiller en vigtig rolle i de psykiske og sociale forandringer, der karakteriserer de tidlige ungdomsår, må det betragtes som særlig alvorligt at føle sig fremmedgjort i denne fase af livet. Denne afhandlings grundantagelse er, at fremmedgørelse er et vigtigt begreb i studiet af børn og unges sundhed og sundhedsadfærd. Det er derfor vigtigt at have gode og velfunderede skalaer til at måle fremmedgørelse blandt børn og unge. Størstedelen af forskning om fremmedgørelse som psykologisk begreb, tager sit udgangspunkt i Melvin Seeman s konceptualisering og inkluderer et varierende antal af hans seks alternative varianter af fremmedgørelse. Tidligere studier har fundet en association mellem fremmedgørelse og alkohol forbrug, fuldskab, pjæk, stofmisbrug og andre former for risikoadfærd blandt børn og unge. Hypotesen i denne afhandling er, at fremmedgørelse øger risikoen for negative sundhedsrelaterede udfald, såsom høj symptombelastning og fysisk inaktivitet. Associationen mellem fremmedgørelse og sundhedsrelaterede udfald blandt børn og unge er dog kun sparsomt undersøgt. Herudover er måling af fremmedgørelse forbundet med flere udfordringer. De skalaer, der er blevet anvendt i børn- og unge forskning, er ikke alle blevet valideret blandt børn og unge. Desuden er der betydelige forskelle i skalaerne og valideringen af disse til trods for den fælles konceptuelle baggrund. Endelig er kun få skalaer blevet valideret i relation til de stigende krav til begrebsvaliditet. Afhandlingens hoveformål var derfor 1) med udgangspunkt i Seemans konceptualisering at udvikle og validere skalaer for fremmedgørelse til brug i sundhedsforskning blandt børn og unge og 2) at undersøge associationen mellem fremmedgørelse og to udvalgte sundhedsrelaterede udfaldsmål: symptombelastning og fysisk inaktivitet. Mere specifikt var formålene: (a) at konstruere og validere et simpelt indeks for generel fremmedgørelse, (b) at udvikle specifikke skalaer for forskellige

5 varianter af fremmedgørelse samt at undersøge den kriterie-relaterede begrebsvaliditet af disse skalaer, (c) at undersøge associationen mellem udfaldsmålet symptombelastning og aspekter af fremmedgørelse målt med indekset for generel fremmedgørelse og (d) at undersøge associationen mellem udfaldsmålet fysisk inaktivitet og forskellige varianter af fremmedgørelse målt ved hjælp af de specifikke skalaer. Afhandlingen er baseret på data fra Skolebørnsundersøgelsen, som er det danske bidrag til det internationale tværsnitsstudie Health Behaviour in School-Aged Children (HBSC), der gennemføres i samarbejde med WHO. HBSC gennemføres som gentagne tværsnitsundersøgelser blandt 11-, 13- og 15-årige skolebørn på tilfældigt udvalgte skoler. I afhandlingen anvendes der danske data fra 1998, 2006 og 2010 fra årige (1998: n=5205) og årige (delpopulation 2006: n=2314; 2010: n=3083) skolebørn. Indekset for generel fremmedgørelse blev konstrueret og valideret i henhold til fire kriterier: 1) teoretisk fundering, 2) interkorrelerede items, 3) korrelation mellem items og udfaldsmål og 4) ingen differentiel item effekt (DIE). I afhandlingens efterfølgende delstudier anvendte jeg item respons teori (IRT) til at undersøge de psykometriske egenskaber af de specifikke skalaer for forskellige varianter af fremmedgørelse. En antagelse i IRT er, at det er muligt at måle et begreb, som ikke kan observeres direkte, ved hjælp af en række items, der er kausalt relateret til begrebet. Det ikke observerbare begreb repræsenteres ved en latent variabel, som kaldes for theta (θ). Varianterne af fremmedgørelse, der måles i denne afhandling er latente variable. I valideringen af de specifikke skalaer brugte jeg Rasch modeller (RM) og grafiske loglineære Rasch modeller (GLLRM) til at sikre den kriterie-relaterede begrebsvaliditet. GLLRM blev også brugt til at generere individuelle θ-estimater, der var justeret for lokal afhængighed (LD) og differentiel item funktion (DIF). Disse θ-estimater blev efterfølgende anvendt i analyserne af associationen mellem fremmedgørelse og fysisk inaktivitet. Afhandlingen vigtigste fund var følgende: (1) Indekset for generel fremmedgørelse målte aspekter af fremmedgørelse og bestod af tre indikatorer på fremmedgørelse: at føle sig hjælpeløs, at føle sig udenfor og at føle en lav grad af fortrolighed med forældre. Målt med dette indeks, var fremmedgørelse signifikant associeret med henholdsvis høj fysisk og psykisk symptombelastning. (2) Det var muligt at udvikle skalaer for to af Seemans varianter af fremmedgørelse (social isolation og normløshed) baseret på tilgængelige items i HBSC 2006 spørgeskemaet, der ikke var udviklet til at måle fremmedgørelse. Begge skalaer indeholdt DIF og LD, men kun i begrænset omfang.

6 Skalaerne fittede begge en RM ved brug af GLLRM. De er derfor essentielt begrebsvalide, essentielt objektive og sufficiente. (3) Det var muligt at udvikle skalaer for fire varianter af fremmedgørelse baseret på items der var inkluderet i HBSC 2010 med det specifikke formål at måle fremmedgørelse. Disse fire skalaer målte fremmedgørelsesvarianterne: magtesløshed, meningsløshed, social isolation og normløshed. De items, der indgik i skalaerne var funderet i Seemans konceptualisering og den umiddelbare validitet (face validity) var god. Alle skalaer indeholdt DIF og LD, men i et begrænset omfang, som det var muligt at justere for ved hjælp af GLLRM. Skalaerne er dermed essentielt begrebsvalide, essentielt objektive og sufficiente. (4) De fire varianter af fremmedgørelse (magtesløshed, meningsløshed, social isolation og normløshed) var signifikant associeret med fysisk inaktivitet. For tre af skalaerne (magtesløshed, meningsløshed og normløshed) var effekten på fysisk inaktivitet ikke-lineær. Associationen blev derfor undersøgt ved hjælp af scatterplots, hvor den totale effekt (odds ratio for fysisk inaktivitet) blev plottet mod den latente skala for hver af de respektive varianter af fremmedgørelse. For magtesløshed og normløshed var associationen stabil ved de laveste θ værdier hvorefter odds ratioen for fysisk inaktivitet steg med en stigende intensitet jo højere grad af fremmedgørelse, der var tale om. For meningsløshedsskalaen var associationen j-formet. Denne afhandling viser, at det er muligt at udvikle begrebsvalide fremmedgørelsesskalaer til brug i sundhedsrelateret børne- og ungdomsforskning. Afhandlingen antyder også, at fremmedgørelse er associeret med sundhedsrelaterede udfaldsmål såsom symptombelastning og fysisk inaktivitet. De fire varianter af fremmedgørelse, der er studeret i denne afhandling, kan bidrage med et øget indblik i psykosociale processer, der kan have betydning for børn og unges sundhed. Begrebet fremmedgørelse kan dermed spille en rolle i fremtidens sundhedsforskning blandt børn og unge.
