OPLYSNINGER OM SUNDHEDSTILSTANDEN / HEALTH INFORMATION: Undertegnede embedsdyrlæge attesterer at/ I, the undersigned veterinary officer certify that: a) Danmark er fri for tegn på vesikulær stomatitis, afrikansk hestepest, infektiøs anæmi, equin viral encephalomyelitis, snive, meliodiosis (Pseudomona pseudomallei), equin paratyfus (Salmonella abortusequi infektion), enzootisk lymphangitis og equin trypanosomiasis/ Denmark is free from evidence of vesicular stomatitis, african horse sickness, equine infectious anaemia, equine encephalomyelitis, glanders, melioidosis (Pseudomonas pseudomallei), equine paratyphoid (Salmonella abortusequi infection), enzootic lymphagitis and equine trypanosomiasis. b) Der har ikke været kliniske eller andre tegn på equin influenza, coital exanthem (dourine), heste kopper, equin viral arteritis (EVA), equin rhinopneumonitis, kontagiøs equin metritis (CEM), kværke og equin piroplasmosis i oprindelsessbesætningen i mindst 6 måneder forud for påbegyndelse af undersøgelserne nævnt i punkt III,f)/ There have been no clinical and other evidence of equine influenza, coital exanthema (dourine), horse pox, equine viral arteritis, equine rhinopneumonitis, contatious equine metritis, strangles and equine piroplasmosis on the premise of origin for at least 6 months before the commencement of the examinations in item III, f). c) Hesten(e) er født i oprindelsesbesætningen eller opstaldet der i mindst 60 dage forud for begyndelsen af karantæne perioden nævnt i punkt d/ The horse(s) have be born on the premises of origin or stabled there for at least 60 days before the commencement of the quarantine mentioned in item d.

3 d) Dyret(ene) har i mindst 7 dage forud for afsendelse til Japan været holdt isoleret på en karantæne stationen, der er godkendt af Fødevareregionen og som udfra et dyresundhedsmæssigt synspunkt er et sikret og garanteret sted. Efter indsættelse på karantænestationen har dyret(ene) ikke været i kontakt med heste, som ikke skal sendes til Japan/ The animal(s) have been isolated in the quarantine station authorized by the Danish Veterinary and Food Administation as a secured and guaranteed place from an animal health point of wiew for at least 7 days before shipment to Japan. After the entry into the quarantine station, the animal(s) have not be in contact with horses which are not destined for Japan. e) Hesten(e) har ikke i løbet af karantæne perioden vist kliniske tegn på equine infektiøse sygdomme ved en omhyggelig klinisk undersøgelse udført af en embedsdyrlæge fra Fødevareregionen / The horse(s) have not shown clinical signs of any equine infectious diseases through a careful clinical inspections conducted by a veterinary officer of the Danish Veterinary and Food Administation during the period of quarantine½. f) Hesten(e) er blevet underkastet følgende undersøgelser med negativt resultat indenfor den periode på 30 dage forud for afsendelse til Japan/ The horse(s) are subjected to the following examination with negative results within the period of 30 days before shipment to Japan: Equin arteritis virus/ Equine viral arteritis : Undersøgelsesresultat og -dato/ Testresults and date of testing Serum neutralisations undersøgelse i en 1:5 fortynding/ Serum neutralisations test at a dilution of dilution 1:5. Kontagiøs equin metritis/ Contagious equine metritis: Bakteriologi på svaberprøver udtaget fra kønsorganer 3 gange med et interval på 7 dage/ Bacterial culture using specimens taken 3 times with an interval of 7 days from genitary organs. (Heste, som aldrig er blevet brugt til parring eller som er kastreret, er undtaget fra bakteriologi)/ (The horses which have never been used for mating or which were castrated are exempted from the bacterial culture) Equin piroplasmose/ Equine piroplasmosis: Mikroskopisk undersøgelse af blod smear og komplementbinding eller indirekte fluoresrescens antistof undersøgelse/ Microscopic examination of blood smear samples and complementfixation test or indirect fluorescent antibody test. g) Hesten(e) er vaccineret to gange mod equin influenza med 4 til 6 ugers interval (eller en enkelt gang ved revaccination) indenfor 1 år forud for karantænen/ The horse(s) have been vaccinated against Equine influenza twice at 4 to 6 weeks interval (or once for booster) within 1 year before the quarantine. Dyret(ene) vil ikke blive sammenblandet andre heste om bord på det skib eller den flyvemaskine, der skal anvendes til transport af hesten(e) til Japan/ The horse(s) are isolated from any other equine animals during the transportation in Denmark. The animal(s) will not be permitted the mix-loading with other horses on board the ship or the aircraft used for the transportation of the exported horse(s) to Japan. j) Foder og strøelse, der anvendes under transport af hesten(e) til Japan, stammer fra samme sted som foderet og strøelsen anvendt i karantænen/ Feed and bedding to be used during the transportation of the horse(s) to Japan come from the same source used for the quarantine. k) Der er ikke ved noget stop under transporten af hesten(e) blevet tilført hverken foder eller strøelse/ No additional feed and bedding have be provided at any port of call throughout tranportation of the horse(s) to Japan. IV. CERTIFIKATET ER GYLDIGT I 10 DAGE / THE CERTIFICATE IS VALID FOR 10 DAYS.
