IV SUNDHEDSOPLYSNINGER / HEALTH INFORMATION Undertegnede embedsdyrlæge attesterer herved vedrørende ovennævnte dyr, at:/ I, the undersigned veterinary officer hereby certify concerning the animals described above that: 1. Dyrene er i dag undersøgt og fundet fri for kliniske tegn på infektiøse og smitsomme sygdomme inkl. afrikansk hestepest, equin encephalomyelitis, infektiøs anæmi, dourine, snive, contagiøs equin metritis og epizootisk lymphangitis./ The animals are today examined and found to be free from clinical signs of infectious or contagious diseases including African horse sickness, equine encephalomyelitis, equine infectious anaemia, dourine, glanders, contagious equine metritis and epizootic lymphangitis. 2. Dyrene har været isoleret under dyrlægetilsyn på en bedrift godkendt af Fødevarestyrelsen fra dato for blodprøveudtagning jf. nedenstående afsnit IV.3 indtil afsendelsesdatoen/ The animals have been isolated under veterinary supervision on a premise approved by the Danish Veterinary and Food Administration from the date of sampling at paragraph IV.3 below until the date of departure. 3. Den (dato), inden for 10 dage før eksporten, er blodprøver taget fra omtalte dyr og sendt til følgende anerkendte laboratorium, hvor de er testet med negativt resultat i hvert tilfælde for følgende: (i) immundiffusion (Coggins) test for infektiøs anæmi. (ii) mikroskopisk undersøgelse af blod film for blod parasitter./ On (date), being within 10 days of export, blood samples were taken from the said animal and sent to the recognized Veterinary Laboratory where they were subjected to the following tests with negative results in each case: (i) immundiffusion (Coggins) test for equine infectious anaemia. (ii) microscopic examination of a blood film for blood parasites. 4. Oprindelsesbesætningen er fri for officielle restriktioner vedr. tilstedeværelse eller mistanke om enhver anmeldepligtig sygdom, som heste er modtagelige for./ The premises of origin is free from official restrictions due to the occurrence or suspicion of any notifiable disease to which equine animals are susceptible. 5. Danmark er fri for afrikansk hestepest, dourine og snive i henhold til OIE s retningslinjer./ Denmark is free from African horse sickness, dourine and glanders in accordance with the guidelines of OIE. V SUPPLERENDE OPLYSNINGER / ADDITIONAL INFORMATION 1. Certifikat er gyldigt i ti (10) dage fra tidspunktet for sundhedsundersøgelsen./ This certificate remains valid for ten (10) days following the date of the health inspection.
