Summary During the past 5 10 years, the interest in the health and well-being of socially marginalized people in Denmark, such as the homeless or heavy substance users, has increased both from a research, practice and political perspective. To a large extent, this growing interest has emerged due to four health studies conducted from 2007 2010 by the National Institute of Public Health, University of Southern Denmark, for the Danish Council for Socially Marginalized People. These studies painted a discouraging picture of a marginalized population having massive problems concerning almost all aspects of health and well-being and simultaneously pointed out the need for further research into a range of poorly explored topics. In particular, the themes of social relations, health and well-being and their interrelatedness called for further, in-depth explorations. International research shows ambiguous and inconclusive results in terms of how social marginalization, well-being and health are interrelated and in terms of providing in-depth understanding of the nature of social relations among marginalized people. The overall aim of this PhD thesis was to increase the understanding of social relations, health and well-being among marginalized people in Denmark using quantitative and qualitative research methods. I explored this aim from both a macro- and micro-analytical perspective. First, by applying a macro-analytical perspective, I examined overall associations between social marginalization, well-being, and self-rated health based on quantitative analyses of a questionnaire-based survey of 1,348 marginalized users of shelters, drop-in centers, treatment centers and social psychiatric centers in Denmark. Second, by applying a micro-analytical perspective, I zoomed in on the social well-being of marginalized people. I explored characteristics of their social relations and their experiences of social isolation based on 46 indepth interviews with users of shelters and drop-in centers. Third, keeping a micro-analytical perspective, I zoomed in on a specific part of social relations; namely, the relationship between parents and children. I explored the characteristics and meaning of parent-child relations, based on in-depth interviews with those of the shelter/center users who had children and who conveyed in-depth stories of relations to their children; in all, 20 out of the 46 interviewees. 1
Based on survey findings, I showed that increased disadvantaged life circumstances (homelessness, substance abuse and poverty) were significantly associated with higher odds of poor self-rated health among men; among women, the association was borderline significant. This was also the case when controlling for age, long-standing illness and mental disorder. In addition, indicators of poor personal and social well-being were significantly associated with higher odds of poor self-rated health among men and women, also when controlling for respondents age, long-standing illness, mental disorder, and number of disadvantaged life circumstances. Based on in-depth interviews, I explored characteristics of the interviewees social relations and their experiences of social isolation. I created a typology of five groups: The socially related and content, the satisfied loners, the socially related but lonely, the socially isolated and the inbetweens. Hereby, I showed that the relationship between social relations and social isolation was complex and did not seem to be related to interviewees degree of social marginalization. In addition, professionals working in shelters and drop-in centers seemed to play a pivotal role, providing social support and help to improve interviewees life circumstances. By exploring marginalized parents relations to their children, I showed that a wider social context of a sociocultural notion of appropriate parenting as well as disadvantaged life circumstances shaped and complicated the frequency of contact and content of parent-child relations. Parents mental well-being and life perspective was strongly embedded in relations with their children. Irrespective of the actual frequency of contact and content of relations, the children brought hope to parents lives. Findings also highlighted the distinctive meaning of parent-child relations to marginalized parents where parenthood paved the way for a sense of normality, recognition, and belonging in society. The combined macro- and micro-analytical perspectives by use of quantitative and qualitative methods allowed for a detailed and nuanced exploration of the social relations, health and well-being of marginalized people in Denmark and nurtured a critical reflection of the empirical findings, the applied theoretical concepts and the methodological strengths and challenges. 2
Empirically, the thesis provided important insight into the social well-being of marginalized people; particularly the detailed nature of their social relations, their experiences of social isolation and the significance of relations for their well-being. The findings underline the meaningfulness of promoting marginalized people s social well-being regardless of their degree of social marginalization and by taking into consideration their different needs, e.g. the need for privacy, or support in reestablishing family contact. Theoretically, the findings point to the importance of viewing social marginalization as a dynamic and multidimensional process in order to capture the complexity of marginalized people s lives. In addition, the findings emphasize the importance of using a more nuanced and contextual understanding of social relations in health research. Finally, on a methodological level, the thesis provided valuable insight into conducting research among vulnerable and hard-to-reach populations. 3
Resumé Interessen blandt forskere, politikere og praktikere for socialt udsattes sundhed og trivsel har været stigende de seneste 5-10 år. Dette skyldes ikke mindst udgivelsen af fire sundhedsundersøgelser gennemført af Statens Institut for Folkesundhed, Syddansk Universitet, for Rådet for Socialt Udsatte i perioden 2007 til 2010. Disse undersøgelser tegner et dystert billede af en marginaliseret befolkningsgruppe med massive problemer inden for næsten alle aspekter af sundhed og trivsel. Undersøgelserne peger samtidig på et væsentligt behov for flere dybdegående studier af en række underbelyste emner, i særdeleshed socialt udsattes sociale relationer, sundhed og trivsel og sammenhængen mellem disse. Den internationale forskning på dette område afspejler en generelt tvetydig og mangelfuld forståelse af, hvordan faktorer som social udsathed, sundhed og trivsel hænger sammen, og hvad der mere indgående karakteriserer udsattes sociale relationer. Det overordnede formål med denne ph.d.-afhandling har været at bidrage til en øget forståelse af socialt udsattes sociale relationer, sundhed og trivsel ved hjælp af kvantitative og kvalitative forskningsmetoder. Ud fra et makroanalytisk perspektiv undersøgte jeg indledningsvis overordnede sammenhænge mellem social marginalisering, trivsel og selvvurderet helbred baseret på kvantitative analyser af en spørgeskemaundersøgelse foretaget blandt 1.348 socialt udsatte brugere af herberger, væresteder og andre sociale institutioner. Ud fra et mikroanalytisk perspektiv stillede jeg dernæst skarpt på udsattes sociale trivsel. Baseret på 46 dybdegående interviews med socialt udsatte brugere af herberger og væresteder undersøgte jeg således beskaffenheden af deres sociale relationer og oplevelser af ensomhed. Endelig fokuserede jeg analysen på en særlig del af udsattes relationer, nemlig de sociale relationer mellem forældre og børn. Det førte til en undersøgelse af karakteren og betydningen af relationer mellem forældre og børn, baseret på interviews med 20 af de i alt 46 interviewpersoner, som havde børn, og som fortalte indgående om disse relationer. De kvantitative analyser viste, at der blandt mænd var en signifikant sammenhæng mellem belastende livsomstændigheder (forstået som hjemløshed, misbrug og fattigdom) og dårligt selvvurderet helbred. Blandt kvinder sås den samme tendens, dog kun tilnærmelsesvis 4
signifikant. Dette var også tilfældet, når der blev justeret for alder, langvarig sygdom og sindslidelse. Analyserne viste desuden, at personer med dårlig mental og social trivsel i højere grad havde et dårligt selvvurderet helbred end personer med bedre trivsel, også når der blev kontrolleret for alder, langvarig sygdom, sindslidelse og belastende livsomstændigheder. Den kvalitative analyse af beskaffenheden af udsattes sociale relationer og deres oplevelser af ensomhed førte til dannelsen af en typologi bestående af fem grupper: de socialt forbundne og tilfredse, de tilfredse enspændere, de socialt forbundne men ensomme, de socialt isolerede og midtergruppen. Analysen pegede på et komplekst forhold mellem det at indgå i sociale relationer og det at føle sig ensom et forhold, der ikke syntes at være knyttet til interviewpersonernes grad af udsathed. Analysen viste desuden, at de professionelle på herberger og væresteder spillede en afgørende rolle i forhold til at støtte interviewpersonerne socialt og hjælpe dem til en forbedret livssituation. Undersøgelsen af relationerne mellem forældre og børn pegede på disse relationers særlige betydning for udsatte forældres trivsel og livsperspektiv. Forældrenes fortællinger om hyppigheden af kontakten med børnene og relationernes indhold afspejlede bestræbelser på at efterleve ideen om det gode forældreskab og den rigtige måde at være forældre på. Livet som socialt udsat, præget af belastende livsomstændigheder, vanskeliggjorde dog i mange tilfælde indfrielsen af disse intentioner. Men uanset graden af kontakt og relationernes reelle indhold bibragte forholdet til børnene og tankerne om børnene forældrene håb: for det første et håb om, at livet kunne blive anderledes og for det andet et håb om at blive anerkendt som forælder. Dermed banede forældreskabet vejen for en normalitetsfølelse, anerkendelse og et tilhørsforhold til samfundet generelt. Den kombinerede makro- og mikroanalytiske tilgang og brugen af kvantitative og kvalitative metoder muliggjorde en dyberegående og nuanceret undersøgelse af socialt udsattes sociale relationer, sundhed og trivsel og bidrog desuden til en kritisk refleksion over de empiriske fund, de anvendte teoretiske begreber og de metodologiske potentialer og udfordringer. Empirisk giver afhandlingen en vigtig indsigt i udsattes sociale trivsel; i særdeleshed beskaffenheden af de sociale relationer, deres oplevelse af ensomhed samt betydningen af relationerne for deres trivsel. De empiriske fund peger på, at det er meningsfuldt at fremme 5
den sociale trivsel, også blandt de allermest udsatte. Dog er det vigtigt at tage hensyn til, at der er forskellige behov: Nogle har mest brug for alene-tid og et privatliv, mens andre snarere behøver støtte i forbindelse med at genetablere kontakten til familien. På et teoretisk niveau peger afhandlingens fund på vigtigheden af at forstå social marginalisering og social udsathed som en dynamisk og flerdimensionel proces for bedre at kunne indfange den kompleksitet, som kendetegner mange socialt udsattes liv. Herudover understøtter fundene, at det er væsentligt at anvende en mere nuanceret og kontekstuel forståelse af sociale relationer inden for sundhedsforskningen generelt. Endelig bidrager afhandlingen på et metodologisk niveau med værdifuld viden om det at forske i sårbare og udsatte populationer, som er vanskelige at nå. 6
